Band 8a Advanced Nurse Practitioner - Palliative and End of Life Care

We are looking for a forward thinking senior nurse who has excellent clinical, interpersonal and leadership skills to lead, develop and support our recently expanded Palliative and End of Life Care teams. You must have a proven track record of progressive leadership and development, be an effective role model and be self-motivated with the passion, drive and adaptability to cope with continuously changing services.

We provide fully integrated 24/7 clinically effective, efficient and high-quality care to people with complex specialist palliative and end of life care needs, based on current evidence and best practice across inpatient and community settings. Our 24/7 service also includes a dedicated End of Life Care team providing rapid response to patients in the dying phase and an overnight service (Pall@Home) to support people to remain in their preferred place of care.

You must be able to provide evidence of a strong commitment to the provision of high quality, evidence based practice in order to enhance the patient and family experience. Visible clinically credible leadership is key in this role and the successful candidate will be someone who encompasses our Trust values and will work with patients, carers and staff to ensure that our services are safe, effective and of high quality.

Main duties of the job

The Advanced Nurse Practitioner is an experienced registered nurse with specialist skills to assess and treat patients and manage the clinical environment, in liaison with the management and consultant team. The nurse will act as an autonomous practitioner in forming clinical decisions and complex management plans in the diagnosis, assessment and treatment of patients. In conjunction with senior nursing and consultant staff formulate the development of clinical guidance for all aspects of clinical practice within the clinical specialty.

The post holder will develop new areas of practice, with nursing and consultant staff ensuring quality standards are met. The post holder will work closely with the interdisciplinary team and collaborate with colleagues to ensure that high quality patient care is maintained.

The Palliative Care team provides specialist information and supportive care to people facing serious illness. This includes symptom control and advance care planning. We recognise that emotional, spiritual, family and financial worries can be just as important as physical problems and we support with these aspects of care as well. Our Palliative and End of Life Care team includes specialist nurses and doctors, pharmacists, social workers, psychological support staff and administrative staff to helpwith the consequences of serious illnesses such as cancer and long-term health conditions. We are able to see patients in hospital, in their own home, and in our outpatient clinics.
