Job summary

Guy's and St Thomas' (GSTT) Intermediate Care Lambeth are looking for a Band 6 Physiotherapist to join our award winning service, to deliver high standard patient care at home, and prevent admissions in to our hospitals.

Main duties of the job

Intermediate Care Lambeth:- Intermediate Care Lambeth team provides healthcare at home to adults aged 18 years and older who live in the borough of Lambeth, South London.- The people that we support have a range of diagnoses and clinical presentations, including falls, orthopedic, respiratory, frailty, amputee, and neurological conditions.

What we do:- Prevent unnecessary hospital admissions.- Facilitate the safe discharge of people from hospital, by providing ongoing healthcare to them at home.- Provide intensive, short term multi-disciplinary rehabilitation and care support to promote functional independence.- Reduce reliance on long term care where possibleHealthcare in the home is rapidly increasing and enables people to be treated at home where they are more comfortable. This helps us to provide a more holistic, well-rounded and bespoke healthcare service to the people we support.

As a member of the team, you will be able to: - Develop your physiotherapy assessment and treatment skills, while utilising rehabilitation support workers. - Develop your skills in complex case management of patients beyond the scope of your traditional Physiotherapy practice e.g. assess for suitable equipment, minor home adaptations, cognitive assessment.- Work in an amazing multidisciplinary team, which is supportive, collaborative, strives to put patient's first and delivers a gold standard community service.- Develop your skills in holistic assessment, problem solving, clinical reasoning, and joint working with other health and social care colleagues, GPs, hospital teams and voluntary sector agencies.- Develop supervisory, leadership and service development skills.- Access appropriate GSTT leadership development training courses to support career development.- Attend structured in-service training: Physiotherapy clinical meetings, as well as Nurse and OT led teachings.- Receive guidance from your very own mentor, monthly supervisions, including an annual appraisal and performance development plan.

Professional / Clinical responsibilities

  • To carry out timely assessment, treatment and rehabilitation of community clients to prevent hospital admission or re-admission and facilitate early discharge home from hospital.
  • To be professionally and legally responsible for own practice and accountable for all aspects of physiotherapy treatment delivered to the client to ensure a high standard of clinical care.
  • To work as an autonomous lone practitioner, case managing clients in the community and coordinating their care.
  • To be able to identify significant health changes that would warrant medical intervention and liaise with the relevant practitioner.
  • To respect an individuals ethnicity, cultural and religious beliefs and adapt your physiotherapy assessment and treatment accordingly.
  • To be competent in using a wide variety of specialised assessment and treatment approaches with adults with a range of conditions, including those with severe physical and cognitive impairment in order to formulate treatment plans, maximise rehabilitation potential and set goals.
  • Use clinical reasoning skills to analyse and interpret referral information, and prioritise these according to urgency of need, risk and service parameters.
  • To demonstrate theoretical and practical knowledge in anatomy, biomechanics, physiology and pathology in the specialised area of care of older people.

This includes:

neurological, orthopaedic, rheumatological and other conditions affecting adults and older people

Physical, psychological, social and cultural factors affecting health and well-being in later life

  • To evaluate client intervention and progress in a timely manner using appropriate outcome measures and evaluation tools. To adapt care plans accordingly and delegate tasks appropriately to RSWs.
  • To discharge clients in a timely manner having reviewed the clients needs and goals, making appropriate onward referrals where necessary.
  • To liaise with the multidisciplinary team and identify when referral on to other professions within the team or outside agencies is indicated. This may include making recommendations on package of care needs when appropriate.
  • To educate clients, carers and family members on the clients medical condition, prognosis and physical management encouraging their involvement and actively promoting healthy lifestyles.
  • To act as lead health professional for clients when required. This may include initiating and co-ordinating multi-disciplinary/multi-agency meetings, liaising with clients/families and onward referrals to other professionals/agencies.
  • To continually update clinical physiotherapy practice in accordance with current research, evidence based guidelines, audit results and government legislation.
  • To maintain accurate and up to date individual client records and ensure safekeeping and confidentiality of these records (including use of the Trust electronic patient records system - Carenotes).
  • To prioritise referrals based on urgency of need and risk according to service protocols. This will require specific skilled physiotherapy as well as multi disciplinary prioritisation techniques.
