The Vital 5 are the main drivers of health inequalities in south east London (SEL). These risk factors link wider social determinants with ill-health. They are high blood pressure, tobacco dependence, unhealthy weight, excess alcohol, and poor mental health.

The purpose of the Vital 5 Check programme is to develop and test a new screening tool for Vital 5 risks. The Vital 5 Check detects Vital 5 risks and initiates management where appropriate (through signposting and referral to intervention services) , prioritising those in greatest need (e.g. Core20PLUS5 groups). The overall goal, therefore, is to increase health equity through targeting the five key drivers of health inequality that are modifiable by equitable health care.

The Vital 5 Check is a SEL-wide programme hosted by King's Health Partners. King's Health Partners (KHP) have formed a partnership with other organisations in order to develop the Vital 5 Check programme, and deliver it as a proof of concept for screening residents across south east London, before embedding into business-as-usual and also expanding to other suitable community locations.

This post is 12 months FTC in the first instance, and could be a secondment.

Main duties of the job

Project Manager (Vital 5 Check)

The purpose of this role is to project manage the SEL-wide Vital 5 Check programme. It will require exceptional collaborative skills and experience of cross-organisational system working.

The postholder will:

  • Have a key role in project management that supports the development of all workstreams. Have specific responsibilities within the SEL-wide Vital 5 Check programme, including collaborating with delivery sites and workstreams

South east London (SEL) is one of the most vibrant and ethnically, socially and economically diverse places in the world, covering the 6 boroughs of Southwark, Lambeth, Lewisham, Greenwich, Bexley and Bromley, with a total population just under 2 million. The SEL Integrated Care System (ICS) and Kings Health Partners (KHP) have launched an ambitious joint programme to improve population health and equalities. We have three areas of work which are interrelated and positively drive (or negatively create inertia); we refer to them as 'cogs':

  1. Population Health Management (PHM): Establish PHM as the embedded way of working in SEL, using data and local insights to improve population health equity.
  2. Prevention and Inequalities (including the Vital 5 and Core20Plus5): Have a relentless focus on the small number of things that disproportionately impact health and health inequalities, starting with the Vital 5 (tobacco dependence, high blood pressure, alcohol consumption, obesity and common mental health conditions)[1]
  3. Make the most of our assets: This includes the 'Anchor agenda' but take us further towards an 'Anchor system' which recognises our assets in the broadest possible sense, including our population, our staff and our organisations.

JOB PURPOSE

The post holder will provide project support to the SEL Vital 5 Check Programme. This is hosted by the Population Health & Equity Programme, which is a joint initiative of Kings Health Partners and the South east London Integrated Care System.

The post holder will also be expected to work with the Programme Manager and programme team to identify and resolve any issues relating to programmes of work. This will include identification and analysis of the nature of the issues and proposals for how they might be resolved.

The post holder will professionally and effectively navigate their area of work at all levels both internally and externally to Kings Health Partners and will also manage and monitor any systems established to manage project requirements and undertake any ad hoc projects as required.

The key performance delivery areas for this role are to:

  • Support and project manage programme wide projects, handling and communicating highly complex and often contentious information between different organisations and audiences;
  • Provide support to the Programme Manager and team on the analysis of key performance indicators for the project associated areas of work and be accountable for achievement of performance objectives;
  • Support the development of strategic business plans across KHP. Work with key KHP leads to identify and support the development of teams;
  • Ensure the effective running of the appropriate meetings within relevant governance structures with proactive agenda, management of attendance, identifying information requirements, gathering of information, data and reports and the scheduling and minute taking of meetings
  • Gather, analyse and prepare project/programme performance and metrics for Executive and Partners Board meetings;
  • Support as required with any budgetary requirements commensurate with the band of the post;
  • Support the delivery of a robust application for bids/funding;
  • Champion a shared information environment within KHP, encouraging sharing and benchmarking of data, etc;

It is expected that the specific programme / projects managed by the post holder will change over time as the portfolio of work changes.

Develop and maintain robust project management and performance management systems for the programme

  • Provide project management advice to members of the KHP executive, programme office and own team.
  • Review and maintain project procedures/activities and associated documentation including storing electronic records of all related documents
  • Ensure correct project management procedures are followed and maintain a record of all project activities.

Project/Programme performance management and development

Providing analytical and business case support to assist in the development of the implementation plans

  • Developing and maintaining gantt chart and or other means of tracking time lines, deliverables and milestones
  • Assisting in the development of template documentation for performance management
  • Advising and supporting in the development of systems and processes to enable their continued progress to include identification, analysis and proposed actions for issues which require alignment or modification to support the work of the Programme.
  • Developing and maintaining electronic libraries for all documentation e.g. strategy and implementation documentation, minutes of key meetings etc
  • Develop and maintain spread sheet or other systems to monitor and manage programme requirements e.g.budgets/reporting for the KHP executive
  • Set up efficient and effective measurement systems and reporting mechanisms to track progress.
  • Provide regular reports and analyses of project/programme progress

Post holders must maintain the confidentiality of information about patients, staff and other health service business in accordance with the Data Protection Act of 1998.
