Job summary

This is a fixed term post for 12 months in the first instance, extendable by mutual consent for those interested in developing a career in cardiothoracic medicine and an interest in medical simulation and education. The trainee would be expected to have a predominantly educational role with responsibility for coordination and provision of undergraduate teaching of medical students, physician associate students, and potentially other members of the multidisciplinary team. The trainee will work to develop and deliver staff training and medical simulation projects. Approximately 1 day per week will incorporate a clinical service commitment alongside specialist training. The trainee will be expected to complete a postgraduate course in medical education.

  • To function independently as a medical simulation and education instructor.

  • To have gained clinical experience in cardiothoracic medicine relevant to higher medical training
  • To have developed an understanding of how simulation training can be used to reduce clinical risk.

Educational Duties of both Posts - Medical Simulation and Education

Harefield Hospital STaR Centre

The STaR Centre at Harefield Hospital is a safe learning environment for students from all clinical disciplines with an interest in cardiothoracic medicine and surgery. Training with the Trust's cardiovascular clinicians, learners can develop professional capabilities, such as:

The well equipped centre incorporates three teaching rooms, a lecture theatre and skills rooms with teaching resources which includes the Apollo high fidelity patient simulator, and surgical simulators including a TOE and VIST interventional radiology simulator. Additionally the centre offers a variety of part task training models. Support is offered to learners and faculty by the centre staff which include an administrator and a technician.

Medical education in the clinical setting is ad-hoc in nature, relying on random exposure to a variety of healthcare problems. Currently, in order to gain experience and become independent health care professionals, doctors and nurses manage patients with close supervision from senior staff.

Medical simulation allows the use of high fidelity human patient simulators where real time evolving clinical situations can be simulated and managed by the appropriate health-care team. Scenarios can be replayed at will and no actual patient harm can occur, thus allowing less experienced health care professionals the opportunity to lead in the management of cases. As well as concrete experience of common medical conditions, medical simulation can provide exposure to important rare and potentially life threatening conditions.

As well as the specific clinical skills and knowledge required to manage a simulated case, the impact of individual and team behaviours on the effective management of medical crises can be explored, facilitated by video assisted debriefing of the scenarios. The debrief is an essential part of the educational experience, facilitating constructive critique and reflection on the management of the case.

International reputation

Students from around the world train at the facility, which has an international reputation for excellence.

The centre has the advantage of having access to the Trusts:

  • pioneering work in the development of artificial hearts
  • dedicated heart attack centre
  • transplant surgeons
  • ECMO

Clinical and educational Duties and Responsibilities

The trainees work includes day to day responsibility and coordination of undergraduate trainees education during their senior clinical attachment. This will include delivering dedicated bedside, lecture based and simulation training sessions. They will also liaise with senior medical staff as needed to facilitate the educational timetable.

The tutor will work to deliver staff training. This will focus predominantly on medical teams but will often extend to nursing teams and allied health professionals.

The Fellow will have approximately 1 day per week which can be focused on clinical care. This will incorporate attendance in cardiology out-patient clinic and an opportunity for development and training in a specialist skill which will further enhance their application for specialist training (eg echocardiography; bronchoscopy; ITU experience). The exact clinical experience developed can be tailored to suit the applicants individual needs.

Educational opportunities

There are weekly clinical meetings in Cardiology and Transplant medicine with educational opportunities including: twice weekly revascularisation, aortic and mitral and weekly transplant MDTs, all held at Harefield Hospital via MS Teams. There are compulsory monthly audit, morbidity and mortality meetings for all staff. We have a state of the art simulation centre where all trainees will be encouraged to develop clinical and practical skills including procedural and emergency scenarios.

Expected outcomes

The successful candidate will be provided with training in the use of simulation equipment and will be encouraged to develop their teaching skills. They will be expected to identify through critical incident review with the risk management team where simulation education may improve patient safety for the future. The Fellow will be expected to lead delivery of existing medical education courses and develop new educational programmes incorporating medical simulation.

The Fellow will be expected to maintain a learning portfolio reflecting upon their progress through the fellowship. The Fellow will be expected to complete a Diploma in education
