JOB DESCRIPTION

This sub-speciality post in cardiothoracic interventional radiology and imaging was created in line with the Royal College of Radiologists Specialty Training Curriculum for Interventional Radiology. The post is for 12 months, to commence in 2022. The post holder will be responsible to the Director of Radiology. The successful applicant will have clinical, research and teaching commitments. This post is designed to give a thorough grounding in thoracic interventional radiology training. In addition, the successful applicant can choose to focus on either cardiac or thoracic imaging at an advanced level. If needed by the applicant, time spent in this post will count towards accreditation. RBHH has a proud record of research in lung cancer, in partnership with the National Heart and Lung Institute (NHLI). NHLI is ranked first in respiratory disease publications, and this is linked to the strong association with RBHH.

Clinical Training

The aim of the post is to ensure the applicant will become proficient in all aspects of the thoracic intervention radiology curriculum including image guided biopsy, drainage and ablation, venous access and stenting, arterial embolization, pulmonary artery catheter directed therapy and the treatment of cardiothoracic postoperative complications. Additionally, the candidate can attain either RCR curriculum level 2 competencies in thoracic radiology or cardiac imaging. Training opportunities will include individualized IR and CT sessions, and participation in MDTs. The post-holder will become proficient at challenging thoracic biopsies, ablation and catheter-directed therapy by the end of the post.

Our new state of the art imaging centre provides a unique opportunity to develop the appointees clinical, technical and management skills in the ambulatory care of cardiothoracic disease including virtual pre-procedure vetting and risk reduction, ambulatory management of thoracic and pleural disorders, virtual patient education, consent and postoperative surveillance. It will be important for the appointee to maintain and develop his/her skills in general interventional radiology. To this end, clinical activity can include gastrostomy insertion, embolization for GI tract bleeding, adult and paediatric vascular access, treatment of visceral and peripheral aneurysms and pseudoaneurysms. If desired, the appointee may elect to retain general interventional radiology skills by an additional rotation in an affiliated general interventional radiology department. To ensure continued proficiency in diagnostic radiology, there will be scheduled plain film, ultrasound and CT sessions and the post holder will be part of a 1 in 7 on-call diagnostic radiology rota.

Research

The post is sponsored by an education and research grant from an industry partner, Cobalt Imaging. The post holder will be given the time, resources and encouragement to undertake research in thoracic imaging and intervention. The unique patient population, imaging technologies, including the new Phillips Azurion image-guided therapy suite and the multidisciplinary environment are expected to enable research work of a high standard. The post holder may apply for the opportunity and funding to present the results of this research at national and international meetings.

The fellow would participate in and facilitate disease specific research projects in lung cancer, opportunities include:

  1. Analysis of outcomes in a high-risk lung biopsy and ablation cohort
  2. Prospective analysis of ablation outcomes in emphysema and interstitial lung disease
  3. Prospective analysis of outcomes for patients referred for a straight to biopsy pathway
  4. Prospective analysis of the use of digital content in the preoperative patient consent process

Teaching

There is a long tradition within the department of teaching in-house medical staff, trainees and overseas visitors. There are many departmental and clinical meetings (Appendix) which the appointee will be invited to attend and, in some instances, oversee.
