Job summary

The EM ACP role is a cutting-edge role in the field of emergency medicine which crosses professional boundaries, undertaking traditional medical roles and leading inter-professional practice. It is a role model of excellence in the delivery of holistic care to patients. tEM-ACP will learn to assess and treat many varied clinical cases. Learning to provide clinical expertise in developing and enhancing the Emergency Department service, demonstrating advanced skills and competencies, and ensuring that evidence-based practice is inherent in all aspects of care and treatment.

Main duties of the job

The trainee Emergency Medicine Advanced Clinical Practitioner (tEM-ACP) will develop generic and specialist knowledge and skills in Emergency Medicine to provided healthcare autonomously and within a multi-disciplinary team with a focus on Emergency Medicine through an established tEM-ACP training pathway.

The post holder will be a 1st level registered practitioner. They will have experience in working within Emergency Medicine. They will have excellent communication skills, and be able to liaise with patients and their families in planning their healthcare needs. They will be expected to provide evidence of their ongoing continuing professional development.

Whilst in training the tEM-ACP will undertake the MSc in Advanced Clinical Practice. As well as the academic pathway the tEM-ACP will follow a consultant-supported tEM-ACP training program (minimum of 3 years / maximum of 5 years) mapped against the Royal College of Emergency Medicine ACP curriculum.

The tEM-ACP will gain increasing skills & knowledge under direct (indirect) supervision of Consultant (or ST4 and above) and / or the credentialed ACP in Emergency Medicine to undertake clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions.

The tEM-ACP will develop advanced skills in clinical decision making, critical thinking, and clinical management skills based upon evidence based practice within their agreed level of specialty competencies.

The RBH is a busy district general hospital serving a population of 600,000 and our ED sees approximately 108,000 patients per year with 25% of these being under 16 years old. Emergency Medicine is at the heart of the hospital's emergency care pathway and provides innovative and high quality care for patients from the moment they arrive at the hospital. It is a consultant led service with consultants providing leadership and patient care on the 'shop floor'. The tEM-ACP post holder will:

Elicit a patient history appropriate to the clinical situation, which will include, presenting complaint, history of the present illness, past medical history, social history, family history, medications, allergies, review of systems, risk factors and appropriate targeted history.

Perform a physical examination tailored to the needs of the patients and the demands of the clinical situation.

Apply comprehensive knowledge, clinical reasoning, and decision-making skills to instigate, evaluate and interpret pertinent investigations.

Initiate appropriate pharmacological therapies applying critical knowledge of pathophysiology, pharmacodynamics, pharmacokinetics, and polypharmacy.

After approval from the Trusts Independent / Supplementary Prescribing Lead, the trainee will undertake Independent / Supplementary Prescribing Module (V300) where after the trainee will record the achievement on their regulatory bodys professional registration. The (t)EM-ACP will then be able to prescribe medications safely and effectively in line with their scope of practice, utilising local or organisational guidelines, national guidelines, and adhering to the Royal Pharmaceutical Society (RPS) Prescribing Standards, and those of their professional regulatory body (e.g., NMC, HCPC, GPhC).

Develop knowledge and skills to enable safe intravenous medication administration.

Demonstrate professional judgement and emotional intelligence to recognise when additional support is required and access the relevant clinical and psychosocial support for self, multidisciplinary team, and service users.

Collaborate effectively with service users and the multi-disciplinary team to continuously assess, evaluate, and adapt management plans to augment care delivery and outcomes.

Demonstrate comprehensive knowledge and understanding of local and national geographical service delivery factors which influence care pathways to initiate and optimise care and outcomes for service users.

Demonstrate an ability to navigate data technology systems to access, correlate and share relevant information in accordance with local and national policies.

Demonstrates evidence underpinning Emergency Medicine capabilities and competencies, through the development of a professional portfolio for Advanced Clinical Practice in line with the RCEM curriculum.
