Job summary

The Palliative Care Support at Home is a community-based service providing palliative and end of life care (EOLC) for people who are approaching EOL and wish to die at home. We provide a flexible and responsive service 24 hours a day, 7 days a week, 365 days a year for patients who are approaching their last hours, days and weeks of life.Patients are registered with a Sheffield GP and varying levels of personalised care is provides depending on the needs of the individual.

Within the Palliative Care Support at Home, you will:-oProvide highly specialised, end of life care support for patients and their families within the patient's home, ensuring high quality individualised patient care during the final stages of life.oSupport the provision of a responsive, flexible 24/7, 365 days a year service.oWork alone, or in 'twos' where required, to deliver clinical and non-clinical care in line with the patient's care plan.oFollow the care plan in the patient's home, escalating any concerns to senior clinicians where a patient's situation changes, or condition deteriorates.oProvide a professional and compassionate service to families, carers, and loved ones to ease the burden at end of life and provide a supportive presence.

Main duties of the job

The Clinical Support Worker (CSW) is part of the multi-disciplinary team delivering care to patients and they will undertake a range of patient care tasks and delegated clinical duties under the direction of a Registered Nurse (or equivalent registered healthcare professional).

To achieve this, the Clinical Support Worker (CSW) will:-oCarry out assigned and delegated clinical care duties as part of clinical team under the direction of registered nurses and undertake some aspects of care without direct supervision.oMaintain clinical skills and competencies of identified skills.oEnsure safe delivery of individualised clinical care is provided to patients and that an acceptable standard is maintained.oUnderstand own level of responsibility, scope of practice and accountability and always act within own capabilities and role.oFollow all STHFT policies and provide a safe environment for the treatment of patients.oParticipate in personal and practice development activities to maintain standards of care.oDisplay a positive attitude towards staff, patients, and relatives at all times, in line with the STHFT's values.

  • Completion of the STH 5-day Healthcare Support Worker Programme within the 12 months prior to employment
  • Care Certificate from an approved awarder
  • GCSE English and Maths at grade D/3 or above (or relevant equivalent qualification or level of ability which would include Functional Skills Level 1)
  • Level 3 Apprenticeship (or equivalent qualification) in Health Care / health related subject OR successful completion of / willingness to compete the trust's required Clinical Support Worker training and competency package.
  • Due to the requirement to undertake visits the post holder is required to have a valid UK driving licence with access to a vehicle or other form of transport, for work purposes
