Are you an experienced nurse with a desire to further your considerable diabetes inpatient knowledge to develop advanced specialist skills in diabetes?
An excellent opportunity has arisen for a motivated and enthusiastic individual to undertake the above role, community post. The post is suitable for those wishing to develop their career as a lead Specialist Nurse. It is a unique opportunity to gain experience in all the services the diabetes team provide. You will provide clinical support and advice to improve the care and management of patients within secondary and primary care.
The successful candidate will work within an established team of Diabetes Nurse Specialists and as part of a wider multidisciplinary diabetes team within STH, to deliver high quality diabetes care. They will provide clinical nursing advice, education and leadership within the speciality working with a high degree of personal autonomy. Optimal care will be delivered throughout all aspects of the patient care pathway. Therefore, an ability to communicate with a wide range of people across different disciplines in both the hospital and community environment is essential.
Main duties of the job
To manage own defined caseload of patients within the agreed scope of practice and taking responsibility for continuity of care acting as an independent practitioner. This will include assessing patients often with highly complex or chronic conditions, providing specialist advice across professional boundaries and planning and implementation of care, running nurse led clinics and comprehensive follow up services; and ensuring a smooth transition for patients through the relevant pathway.
To maximise efficiency and quality of care for individual and groups of patients, acting as anadvocate through the patient pathway where necessary.
To participate in the diabetes centre cover, which offers advice to patients, primary care Providers. This service offers urgent appointments to patients who are new to insulin.
To actively develop change through service audit and evaluation in a way which promotes change for the benefit of patient care within the objectives of the business and service plan.
