Job summary
To manage workload given by a senior Clinical Nurse Specialist without direct supervision, using own judgement & discretion, working towards autonomous management of nurse led clinic.
To optimise the continuity of care of patients between the Hospital and Community through providing opportunities for liaison between all those involved in the health care of patients.
To develop relevant knowledge related to investigations and treatment options in order to provide patients with necessary information to ensure informed consent.
Participate in effective nurse-led care, including speciality clinics where appropriate. These may be held in the Trust or in community clinics such as sports centres.
Participate in clinics, advising the patients on individual pain management strategies as appropriate such as the use of TENS, medication review and advice, ensuring that precautions and contra-indications are highlighted, and providing back up support and information when required.
To manage own case load of patients under the supervision of the Senior CNS team, developing individual agreed management regimes with the patient. These will be responsive to their psychological, physiological and educational needs.
A commitment to achieve the following within 24 months in order to progress to Band 6 CNS - The Band 5 - 6 competency framework.
Main duties of the job
To deliver high quality patient care, acting as a role model for nursing practice across the specialty and Trust.
To provide clinical nursing advice and education within Chronic Pain Services.
To act as a source of advice and knowledge for patients, carers, and healthcare professionals.
To work within the multi-disciplinary team in the provision of optimal care throughout all aspects of the patients care pathway and in all clinical settings.
