Diabetes Nurse Specialist Community

Job summary

An exciting opportunity has arisen for 2 Diabetes Nurse Specialists to support our Community Nursing teams to effectively manage the care of diabetic patients on their caseloads across Sheffield. There are twenty two Community Nursing Teams delivering close to 400 visits per day to patients to administer insulin. This role will be instrumental in exploring innovative approaches to reduce demand on the nursing service whilst ensuring optimal patient management.

Main duties of the job

The successful candidate will work alongside the Community Nursing teams providing diabetes education to the staff, undertaking individual patient reviews, and developing best practice across the service.

The successful candidate should have experience with diabetes patient care and experience of working within the community.

  • Valid 1st level registration (NMC)
  • Accredited qualification at diploma level or above in diabetes or equivalent experience
  • Degree level education or working towards
  • To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.
  • Independent nurse prescriber or equivalent or willing to work towards
  • Evidence of advanced training in diabetes

  • Significant relevant post registration experience
  • Experience working in a primary/community nursing setting
  • Extensive, demonstrable post registration experience in the speciality including experience/knowledge of service management, planning and delivery
  • Experience of managing change within the speciality
  • Evidence and experience of innovative working in diabetes
  • Evidence of working effectively and collaboratively across professional and organisational boundaries
  • Experience of autonomously managing patients with diabetes within a defined caseload
  • Experience of caseload management, health assessment and advanced skills in patient education
