Active Programmes Nurse

Job summary

To assess, diagnose, plan and develop specialised rehabilitation programmes for patients referred to the Active Programmes team.

To supervise/train inexperienced nurses/assistants/students working within the service.

Deliver individualised cardiovascular programmes of care (for example phase 2, Heart Failure Angina, AF) using in-community programmes, or by self-directed manuals (where appropriate) or digital solutions or home visits.

Deliver individualised pulmonary rehabilitation programmes following assessment to ensure continuity of service provision to all patients referred to Active Programmes.

To use continuous assessment to evaluate treatment outcomes and adjust treatment plans accordingly, appropriately referring to other agencies to meet patients' needs and promote integration of services.

Main duties of the job

  • Assess, plan, implement and evaluate individualised programmes of care.
  • To receive and communicate complex, sensitive or contentious information to patients and carers, who may have barriers to understanding eg dysphasic, complex mental health problems, to maximise rehabilitation potential and to ensure understanding of conditions.
  • To have expert verbal and non-verbal communication skills of motivation, explanation and reassurance to agree and gain co-operation of patients and carers in their treatment programmes.
  • To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of rehabilitation and to ensure a consistent approach to patient care and treatment e.g. moving and handling.
  • To provide nursing advice, teaching and training to other members of the MDT regarding the management of patients and the role of nursing.
  • To train, supervise and performance manage junior staff, senior assistants and assistants and students in a clinical, professional and organisational capacity. This may include undertaking formal appraisal.
  • To assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment.
  • To be responsible for ensuring the effective selection and use of all treatment resources.

This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being externally funded.
