Job summary

This is an exciting opportunity for enthusiastic, highly motivated and dedicated Advanced Clinical Practitioners to join our team at Sheffield GP Collaborative.

GP Collaborative provides provides urgent primary care for the city of Sheffield and clinicians will be required to work autonomously providing face-to-face consultations. telephone triage and home visits, to assist with clinical audit and other managerial responsibilities as directed from the Clinical Team. This involves working with adults and children in an out of hours setting including evenings, weekends and bank holidays.

Main duties of the job

  • To work as part of the unscheduled care team delivering a high standard of care to patients in all clinical areas within the GPC community Out of Hours Service.
  • To work autonomously providing a holistic approach to the care of patients through the appropriate method of assessment, planning care and treatment or referral as appropriate across a wide spectrum of clinical conditions within the unit. The role also requires the practitioner to work as an effective member of the team within the unit.
  • To be a role model providing clinical leadership/mentorship as a resource to all ACP's and trainees, and GPs within the GPC, encouraging and developing best practice.
  • To lead and maintain clinical governance and risk management initiatives with each unit, ensuring relevant processes are maintained within the service.
  • To hold a good working knowledge of current legislation surrounding advanced practice, and non-Medical Prescribing, including best practice in the directorate of Unscheduled Care.

The Combined Community & Acute Group is one of the largest of ten Care Groups in Sheffield Teaching Hospitals NHS Foundation Trust (STHFT). This provides the opportunity to develop new integrated working which the Care Group is actively pursuing.

The GP Collaborative provides an urgent Out of Hours primary care service for the city of Sheffield. This includes providing clinical triage for patients referred by the 111 service, face to face clinical assessment at the GP Collaborative as required and home visits for the house bound and those at the end of life.

The GP Collaborative also provides Primary Care streaming of patients attending ED at the Northern General Hospital and at Sheffield Children's Hospital with primary care needs that can be redirected and seen in the GP Collaborative.

  • Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC with relevant clinical experience
  • Willing and able to successfully complete a full Masters in Advanced Clinical Practice - which may be via the apprenticeship route
  • Post Graduate qualification in Advanced Practice
  • Advanced Physical Assessment module, Non-Medical Prescribing (if part of proposed role)
  • Advanced life support qualification (ALS/ILS) or willingness to undertake
  • Evidence of relevant continuous professional development

Desirable

  • Recognised Teaching & Assessment Qualification or equivalent experience
  • Recognised Leadership Award/qualification or equivalent experience

  • Experience of working in an extended role with experience of autonomous practice
  • Evidence of portfolio of practice and sign off of competence
  • Experience of delivering training / education to others
