An exciting opportunity has arisen for highly motivated and enthusiastic nurses to join our team in the Ambulatory Wound Care Clinics.
Ambulatory Wound Care Clinics are based in the Community and provide assessments and ongoing treatments to patients who are ambulant and not housebound who have lower leg problems and wounds.
The service amalgamated with the Lymphoedema service in 2021 and works collaboratively ensure better patient journey and outcomes. We work with Tissue Viability, Dermatology, Vascular, Podiatry and Lymphoedema liaising closely with GP's, Primary Care and Community Nursing.

Main duties of the job
To support the day to day running of the nurse led clinics Sheffield.
To be responsible for the assessment and management of the health needs of an identified population and for the development, implementation, and evaluation of programmes of care for patients with lower leg problems and ulceration.
To provide support to the team to include clinical audits, education and training.
To work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the population.
To assist in the Duty co-ordinator role in the care delivery of the service needs to include daily triage of all new referrals and prioritise clinical queries according to clinical needs.
To provide high quality patient services and care for the directorate.

One post (0.6 WTE) is permanent and one post (0.8 WTE) is advertised on a fixed term contract basis for a period until end January 2025 due to the post being recruited as part of a limited term project within the Trust. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the requirement for the post to be maintained as a part of the project's implementation and the timescales associated with this.
