Job summary

An exciting and rare opportunity has arisen for a highly motivated and innovative nurse, physiotherapist or paramedic with advanced clinical skills to be recruited to a community Consultant Practitioner post. We are seeking a full-time, experienced, outstanding Advanced Clinical Practitioner who will work with and support the multi-professional team in delivering high-quality, personalised care for patients in their own homes.

Main duties of the job

The role provides a great development opportunity for a Community Consultant Practitioner to provide clinical leadership to the Advanced Clinical Practitioner team including assuming overall clinical responsibility for patients on the Frailty Virtual Ward. The post holder will also have the clinical lead role in further developing our Same Day Urgent Community Care portfolio, e.g. Urgent Community Response.

This is an exciting and demanding role, which will require effective and compassionate clinical leadership, to collaborate with experienced colleagues and to manage competing priorities. The post holder will have excellent interpersonal skills, and be resilient. The post holder will work closely with the Head of Integrated Community Care, the Clinical Director, and develop effective partnerships with Geriatricians, General Practitioners and other professionals to deliver high quality out of hospital personalised care.
