Job summary
An opportunity has arisen for two dynamic, autonomous, forward-thinking ACP to join our Critical Care Outreach team (CCOT).
The service works closely and collaboratively with ward teams assisting in the delivery of timely support and management of acutely ill/deteriorating patients. This includes attending cardiac arrests and the follow up of patients discharged to the wards from critical care. The CCO team are also involved in regular trust wide education and training programmes delivered to ward based teams. The main aim of the role is to avert admission to critical care by timely recognition and intervention or to ensure safe transfer to a higher level of care that can facilitate appropriate treatment.
You will be expected to develop maintain and expand your professional portfolio in line with the four pillars of advanced practice.
Main duties of the job
To work autonomously offering specialist advice and support to facilitate high quality, individualised care to acutely ill/deteriorating patients beyond the critical care environment. You will:
- Undertake comprehensive A-E assessments of acutely ill/deterioratingpatients. Formulate a plan of care and implement appropriate interventions to enhance patient safety and prevent further deterioration. Evaluate the effectiveness of interventions and reassess at appropriate intervals.
- Provide specialist clinical support and guidance to all matters concerning critical care outreach
- Participate as a member of the cardiac arrest team and act as a role model.
- Facilitate and provide trust wide education and training in the form of the deteriorating patient study day and SHARxP.
- Participate in the promotion of the critical care outreach service, developing a rapport with the wider MDT to encourage appropriate and timely escalation to the outreach service.
- Work collaboratively and communicate effectively with the wider MDT as well as specialist registrars and consultant intensivists within critical care.
- Provide advice, care and support to patients recently discharged to the wards from critical care. This may include advice regarding psychological services available.
- Be independently motivated as well as motivated within the team to work towards a shared vision in developing the CCO service.

Current working hours are 8-6pm Monday to Friday. We are working towards expansion of service cover to 24/7 including rotation through unsocial hours cross city.

- Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC with relevant clinical experience
- Willing and able to successfully complete a full Masters in Advanced Clinical Practice - which may be via the apprenticeship route
- Post Graduate qualification in Advanced Practice
- Advanced Physical Assessment module, Non-Medical Prescribing (if part of proposed role)
- Advanced life support qualification (ALS/ILS) or willingness to undertake
- Evidence of relevant continuous professional development including Critical Care Step 1 Competencies or equivalent, Critical Care course modules.
Desirable
