An essential part of the Community ACP role is the ability to work autonomously, using own initiative and clinical reasoning in a variety of care settings. Whilst being able to demonstrate the ability to prioritise workload, provide support and education to the wider MDT.

The Community ACP will work within the Integrated Care Teams and Virtual Ward as a resource to provide clinical information, to support the team with case management, care planning, interventions and monitoring of patients with complex needs including patients with multiple complex conditions.
