Job summary

An exciting secondment opportunity has arisen within the Intensive Home Nursing Team for a Band 5 Nurse delivering high quality patient care within palliative care. The service which is community based in the Michael Carlisle Unit in the Southwest of Sheffield provides palliative and end of life care (EOLC) for people who are approaching EOL and their preferred place of care is at home.

This post will provide leadership which will help shape the service provision for the future. This role requires close working relationships with patients, families, Continuing HealthCare, Community Nursing, Discharge teams, Hospices and Social Care Providers.

Some hands-on clinical care will be required and you must have a valid UK drivers' licence and have a vehicle for work purposes.

Main duties of the job

We are looking for an experienced community based staff nurse who will enjoy the diversity of this challenging role, splitting time between supporting IHNS support staff in the patient's home, and being office based. The role of the staff nurse will be to review patients that are on both services, on a daily basis, to ensure that the patients fulfill the service criteria.

The role will also be to provide support and training to staff in the patients home, checking documentation and ensuring quality care. It is also expected that the post holder will assist in the day to day running of the busy office. There will be some supervising of band 2 support workers and they will be responsible for a number of Band 2 support worker appraisals.

Due to the requirement to undertake visits the post holder is required to be a driver with a valid driving licence.

This vacancy is advertised on a fixed term contract basis for the maximum period of 12 months. Where there is potential, the secondment/fixed term contract may finish early, or in the first instance, the secondment/fixed term contract may be extended due to the internal/external secondment of the substantive post holder. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and secondments in relation to the substantive post holder.
