Main duties of the job

The South East (SE) Neurosciences and Trauma Strategic Network is seeking a Complex Case Manager (CCM) to support the complex rehabilitation ODN and the Strategic Network in reviewing and transforming needs based complex rehab pathways across the South East.

The successful candidate will work with 2 existing CCM's, with this role having a particular focus across Hampshire and Isle of Wight.

This role is unique in that it supports patients and their families in navigating the highly complex rehabilitation pathways and supports the Strategic Network in its transformation / streamlining of the pathways to improve access and outcomes for complex rehab patients.

This roles reports to the lead of the Neurosciences and Trauma Strategic Network and will support the complex rehab pathways across all the ODN's within the Strategic Network.

Qualifications, knowledge and experience

Essential

  • First degree or equivalent
  • Registered AHP, RN or other health or social care professional, with extensive knowledge & experience of complex specialised SCI and specialist rehabilitation services.
  • Evidence of post qualifying and continuing professional development to include extensive knowledge of specialist areas, acquired through post graduate studies or equivalent experience plus further specialist knowledge or experience to master's level / equivalent
  • Member of relevant professional body
  • Masters or equivalent experience
  • Specialist qualification
  • Leadership, or management training, at master's level or equivalent experience
  • Consolidated experience to demonstrate capability and capacity for management of acute health services at a senior level
  • Extensive knowledge of National and Local NHS and N&M agenda
  • Extensive experience in the acute healthcare setting in clinical leadership/clinical management position
  • Demonstrates experience of managing challenging problems by developing practical workable solutions to address them.
  • Ability to communicate vision to staff and manage change effectively.
