Are you looking for a new challenge? The Thoracic Surgery Nurse Case Manager (NCM) team is looking for a motivated, flexible and hardworking individual to join this progressive team.
University Hospital Southampton is a tertiary referral centre of excellence for Thoracic Surgery that works in collaboration with hospitals across the south of England. The Thoracic Surgery NCM role is an integral part of both the medical and wider health care team, involved in all aspects of patient care in the work up to surgery. The NCM role embodies the Trusts core values of patients first, working together and always improving.

WHAT YOULL DO:
The Nurse case Managers provide support for patients from pre-operative pathway. You will liaise closely with other nurse specialists and MDT teams across the regions to ensure quality care throughout the whole of the patients journey. You will undertake holistic nursing and medical assessments of Thoracic Surgical patients, including history-taking and physical examination, in the outpatient setting. You will also perform pre-admission assessment and clerking in preparation for surgery.
WHAT WERE LOOKING FOR:
The post holder will ideally have experience of supporting patients with Cardiovascular & Thoracic disease within primary or secondary care. They will need to be adaptable and flexible and work in varied environments across organisational boundaries. You will require a flexible approach to working patterns.
You will be expected to have excellent communication skills, promote the delivery of a high standard of patient-centred care and work between organisational boundaries. You will analyse, monitor, and implement actions that ensure the service delivery is safe and effective in line with the Trust values with the support of your colleagues and line manager.
