Main duties of the job

The Patient and Family Support Hub (PFSH) Healthcare Support Worker (HCSW) is part of the multidisciplinary team that delivers care to patients, relatives, and carers in the PFSH. To achieve this, the HCSW will assist in the provision of high-standard individualised care, under the supervision of the staff working within the PFSH team.

a) To support the Patient and Family Support Hub Coordinator and Manager in leading the development and delivery of the Patient and Family Support Hub.

b) To support projects for key initiatives, reporting to the Patient and Family Support Hub Coordinator or Manager.

c) To help high-risk and vulnerable patients, direct them to relevant information and volunteer assistance or escalate concerns to the Patient and Family Support Hub Coordinator or Manager.

d)To support the volunteers and guests in the Patient and Family Support Hub.

The successful candidate(s) will work Monday-Friday initially over a 8 am-4 pm rota and must be able to work on-site in support of patients, relatives and carers that visit or use the Patient and Family Support Hub

1. Assist patients with activities of daily living, this may include all basic care such as assisting with toileting needs, and repositioning of the patient whilst in the PFSH or when retrieving them from a clinical area.

2. Respect and maintain the patients privacy and dignity at all times, whilst acknowledging the personal beliefs and wishes of patients. Taking into account communication barriers, communicating effectively to deliver person-centred care.

3. Communicate regularly with the patients, their relatives, carers and the multidisciplinary team (including volunteers) involved with the provision of care in a caring, compassionate and professional manner.

4. Share responsibility for key aspects of housekeeping and stock control. Demonstrating a good awareness of cost efficiency and ensuring that the PFSH environment remains a safe, clean and tidy area for both staff and patients.

  • Completed care certificate or ability to complete within 6 months of appointment.
  • Completed secondary education

Knowledge and experience

  • Evidence of empathy, patience and dependability
  • Ability to deal with a range of difficult situations
  • Be able to take instruction / directions and work within the confines of the role
  • Ability to cope with fluctuating demands and pressures of the role.
