Main duties of the job

A unique and exciting opportunity has arisen to lead and shape a new service at UHS, building on the success of the current Patient Support Hub, the new 'Patient and Family Support Hub' will be situated within repurposed estate on site at UHS, the role will be to manage the Hub through a redesign and ensuring that well being and key support services are available to our patients and their families. It is essential that the successful candidate has a passion for patient and family experience and supporting the clinical teams at UHS.

Job responsibilities

This role will focus primarily on:

Provision of a multi-purpose Patient and Family Support Hub for all patients, carers and loved ones to address and reduce both health and disease inequalities. Creation of a central point for patients, carers and loved ones to come for support when requiring additional support to improve their accessibility to Trust services. Improvement of wellbeing support for patients on the waiting list through the waiting well service, creating equitability in access to counselling and education on coping mechanisms. Access to respite services for unpaid carers to improve the provisions available to them and to support carers in caring for their loved one. Reduce the dehumanisation of hospital-bound patients, bringing the outside world into the hospital with services such as a hair salon, nail bar and a mock living room designed to boost patient wellbeing and help them to feel more recognisable. Provision of a private space for long-term stay or end-of-life patients to ensure that there is room away from the clinical environment, supporting a positive palliative care experience. Access to a private room specially designed to be learning disability and autism friendly to create space for patients. Improve access to community support for patients and loved ones with drop-in sessions and signposting available for services such as Citizens Advice or community support services.
