Job summary

An exciting opportunity has arisen to join the expanding Integrated COPD Team at UHS.

We are looking for dynamic, enthusiastic and committed Senior Healthcare Assistant to support the delivery of our COPD service and the development and delivery of our targeted lung health programme.This is an ideal opportunity for those who want to develop or further enhance their specialist knowledge and skills within a supportive environment.

The Integrated COPD Team provides specialist evidenced based care and support for patients with COPD within the acute hospital setting and on-going support, monitoring and education within their homes to promote independence to manage their disease within primary care.

WHAT YOULL DO

In this role you will:

Establish contact with patients/carers from the time of initial referral or identification to provide specialist nursing advice and practical and psychosocial support.

Communicate to patients in a way that empowers them to make informed choices about their health and health care.

Manage patient care to ensure patients receive care that works in partnership with other healthcare professionals including referrals to alternative care pathways and treatments as clinically indicated.

We provide UHS and community Clinics which you will help run with the ICOPD Respiratory consultants

WHAT WERE LOOKING FOR

As a Respiratory Health Care Assistant within the Integrated COPD team you will be expected to have excellent communication skills, promote the delivery of a high standard of patient-centred care and work between organisational boundaries.

You will be expected to work across the city with the support of the Senior Sister and ensure the service delivery is safe and effective in line with the Trust values.

The post holder will require a flexible approach to working patterns, but will typically cover shifts from 10-6pm.

The post holder will need to be adaptable and flexible and work in varied environments on multiple sites and in the community.

The post holder will require access to a vehicle and have a valid driving licence due to cross-site working.
