Job summary

An exciting opportunity has arisen in the established Older Persons' Specialist Team (OPST) within Division B - Medicine for Older People, for a permanent Band 6 Older Persons Specialist Practitioner. The successful applicant will be joining a team of highly motivated, experienced, and respected specialists.

The post will be based with the OPST but working alongside the medical teams to support patients over 80 who have complex health and social care needs.

This will be a permanent position working 28 hours over Monday- Friday.

Within the scope of the role, the Older Persons Specialist will assist the medical and nursing teams in managing caseloads of patients who have complex health and social care needs and work closely with the community teams in the management of long term conditions. This is an innovative opportunity for integrated working across both primary and secondary care providing a high quality, evidence based seamless service for patients.You will provide expert clinical knowledge and support to both the medical and ward/department teams with the management of these groups of patients. You will work autonomously within the structure and requirements of your allocated team and make key decisions regarding your patient caseload.

  • Registered professional on relevant part of the NMC/HCPC register
  • Degree qualification, 120 credits at L6 or possess an equivalent knowledge to be assessed through evidence of relevant specialist training, experience, or portfolio
  • Post registration qualification within area of speciality.
  • Completion of all Band 5 (preceptorship) competencies
  • Higher Education Institute approved course in supporting learners in practice or evidence assessed to be of equivalent experience, demonstrating support to a range of learners in practice. Provide evidence of- oSupporting a range of learners in practice oSupporting colleagues to promote effective learning in practice

  • Registered on MSc Pathway - completion of HTPA Competencies
