Job summary
An exciting opportunity has arisen in the established Older Persons' Specialist Team (OPST) within Division B - Medicine for Older People, for a permanent Band 6 Older Persons Specialist Practitioner. The successful applicant will be joining a team of highly motivated, experienced, and respected specialists.
The post will be based with the OPST but working alongside the medical teams to support patients over 80 who have complex health and social care needs.
This will be a permanent position working 28 hours over Monday- Friday.

Within the scope of the role, the Older Persons Specialist will assist the medical and nursing teams in managing caseloads of patients who have complex health and social care needs and work closely with the community teams in the management of long term conditions. This is an innovative opportunity for integrated working across both primary and secondary care providing a high quality, evidence based seamless service for patients.You will provide expert clinical knowledge and support to both the medical and ward/department teams with the management of these groups of patients.
