JOB OVERVIEW

Wessex Integrated Stroke Delivery Network (ISDN) is one of twenty in the country established to lead the transformation of stroke services in line with the ambitions laid out in the NHS Long Term Plan.

The network is looking for a highly skilled and motivated manager to drive the next phase of its work across Hampshire & Isle of Wight and Dorset. The ISDN Manager will work alongside 3 clinical leads and a project assistant to ensure delivery of objectives across the whole pathway of care from stroke prevention, through hyper acute and acute care, rehabilitation, and life after stroke.

The post holder will work autonomously but with and through the range of staff and teams delivering stroke services, including those in health and social care, the voluntary sector and patients and carers. Whilst the ISDN is hosted by UHS it is accountable to the two Integrated Care Systems in HIOW and Dorset and relationships with the two systems will be a critical success factor for ensuring sustainability of commissioning and delivery the transformation of stroke services.

The post provides an exciting opportunity to facilitate and support a high functioning, networked approach to the delivery of evidence-based stroke care across Wessex.
