Job summary

We have an exciting opportunity for 2 full time (12 months Fixed Term Contract or secondment with agreement from your current manager) highly experienced and autonomous registered nurses with advanced skills in managing complex frailty, long term conditions and caseload management with a V300 prescribing and appropriate Specialist Practice qualification. You will be expected to proactively case manage integrated care for complex patients in the community, working closely with multi disciplinary teams to improve patient outcomes, reduce hospital admissions and facilitate early hospital discharges where it is appropriate to do so. Experience in district nursing/primary care nursing is desirable.

You will work autonomously, making differential diagnoses, prescribing treatments/investigations and/or referrals and possess a high level of communication and interpersonal skills at inter-professional and inter agency levels to ensure that high standards of care pathways are maintained.

One post will be based in Nottingham West (Stapleford) and one in Nottingham East (Arnold). There is an expectation that the successful post holders will be required to work within the Community Matron team across the South of the County and provide cross cover when needed.

Main duties of the job

  • Provide proactive, integrated care for complex patients in the community, improving patient pathways and reducing hospital admissions.
  • Manage a complex caseload, undertaking comprehensive holistic assessments and developing proactive care plans.
  • Work autonomously and collaboratively within a Multi-Disciplinary Team (MDT) to manage complex frailty and long term conditions.
  • Utilise advanced clinical skills, includingV300 prescribing for complex case management.
  • Lead on service improvement initiatives and provide clinical education/support within your local nursing teams.
  • Educate patients, families, and other professionals on managing frailty and long-term conditions.
