Job summary
This is an exciting opportunity to be part of the Intensive home treatment team as a Community Support Worker within the Broxtowe area if you are seeking to develop your skills and expertise in working with clients aged 65 years and over living in the local community and experiencing severe mental health problems. You will be an integral member of a dynamic multidisciplinary team offering creative and consistently high quality evidence-based interventions

Successful applicants with no prior NHS experience would normally be placed at thebottom of the band in line with Agenda for Change. This salary is below the minimumsalary required for sponsorship for a Skilled Worker / Health & Care visa. In thesecircumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker/Health & Care visa.

Main duties of the job
As part of the intensive home treatment team you will be expected to deliver intense input within the persons home. This service works within the older adult population whom often present acutely unwell and require an intense care plan in hope of avoiding hospital admission.
You will contribute to assessments and interventions, undertaking nursing, therapy and social activities as delegated by the relevant profession. You will frequently work without direct supervision in the community and be required to work in a flexible and holistic way in order to respond to this client group's needs. You will promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
We are seeking enthusiastic team players, to work within a dynamic multi-disciplinary health team, with excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
