We are looking for an experienced Learning disability nurse to join our Primary care liaison Team in the County of Bassetlaw.

Be part of a team that breakdowns barriers that people with Intellectual Disability experience when accessing primary health care services and support Intellectual disability patients to access National Screening programmes.
Have an active part in delivering enhanced service training to primary care colleagues and supporting the GP Practices in keeping their Learning Disability Registers up to date.
The post is a fixed term part time position.
22.5 hours a week.

Main duties of the job
Liaise with Primary Care, IDD Community Services, Patients, carers and family to help facilitate and promote reasonable adjustments required for people with a Learning Disability to access Primary Health Care.
To educate primary care professionals regarding the Health Needs for people with Learning Disability
To work alongside people with a learning disability so that they are able to access and understand primary care they require.
Support patients to access mainstream screening services.
Please note that this post does not meet the pay or skill level required for a Skilled worker visa.
Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimumsalary required for sponsorship for a Skilled Worker / Health & Care visa.
