Job summary

An exciting opportunity has arisen for a Band 6 Clinician to join the newly formed Clinical Access Line and County South Crisis team. This newly developed role consists of working across both services as a crisis care practitioner.

The Clinical Access Line is a new service, that is central to the delivery of the 111 Mental health option national requirement. Within the Clinical access line, there will be recovery workers who will be taking initial calls and directing them to the most appropriate service to meet their needs. The team will also provide advice and guidance through a triage process where the urgency of care required is assessed and signposted accordingly. Relevant training will be provided upon successful recruitment to the post.

Within the County South CRHT the post holder will also have devolved management, leadership and development role in ensuring and supporting the effective delivery of clinical practice. This service supports clients aged 18 and over with severe mental health problems and/or an acute psychiatric crisis in accordance with quality practice and procedures. The post holder will provide mental health assessments and support to clients within a variety of settings including the clients home, and other health and non-statutory setting.

You will be part of a team that consists of both qualified and unqualified clinicians who are knowledgeable about different services and options and you will be supported by a Clinical lead on each shift.

Main duties of the job

We are seeking enthusiastic and experienced Band 6 Mental Health clinicians to provide clinical assessment, triage and signposting to citizens of all ages in a mental health crisis.

You will be an experienced Clinician and will have proven post qualification experience. You will have a sound knowledge of risk assessment and demonstrate sound evidenced based practice. Experience of working in a crisis team or community setting will be advantageous.

The post holder will participate in the delivery of effective clinical services within the Crisis Resolution/Home Treatment Service in line with specified national and local standards.

The access line will provide one number for support and referrals to secondary mental health crisis services within Nottingham and Nottinghamshire and will include all ages.

To support and supervise recovery workers taking calls and to assist in making appropriate decisions of signposting. Also to plan, allocate and supervise the work of the team.

To complete triage assessments of more complex calls using a triage tool to assess the urgency of care that is required.

To be able to work alongside partnership agencies and services such as EMAS services and street triage as well as the crisis resolution teams.

To demonstrate expertise in analysing and reflecting upon their own clinical practice and that of others in all aspects of care given to patients in crisis and their families/carers.

To work effectively with colleagues from other disciplines as appropriate throughout the assessment and treatment process.

To have highly developed report writing skills and be able to make appropriate referrals to other services.

Be responsible for providing and receiving highly complex, sensitive distressing and emotional information in relation to health issues.

Developed knowledge of safeguarding procedure and experience in how to manage these situations including advising colleagues where necessary.

To work within professional and ethical frameworks established by national bodies such as the Department of health and the NMC

To complete home and community visits for clients. These visits would consist of comprehensive assessments, reviewing risks thoroughly and on a regular basis.

The post holder would be expected to travel independently to visits throughout a variety of shifts.

The post holder would also be expected to feel competent in assessing clients suitability for admission to psychiatric hospital and feel confident in completing gatekeeping assessments in the community.
