Job summary
This new role within our pre existing Pulmonary Rehabilitation (PR) team, plans to provide greater ease to the patient pathway through PR.
The Pulmonary Rehabilitation service delivers individual exercise in a group / class setting to patients referred to the service with a respiratory condition. Providing a triage, individual pre assessment and post PR assessment to ensure appropriate referral and to gain patient outcomes.
Integral to the service is entering data for a national audit programme. pulmonary rehab runs on an 18 week waiting pathway.
The role of the assistant practitioner is patient facing and delivering post PR assessments once competencies have been completed, to assist and deliver group based exercise and educational programme twice a week over an 6 week period at a variety of sites across the mid Nottinghamshire.
Main duties of the job
The role is primarily concerned with delivering group based educational information to people of all ages diagnosed with dementia across the county. This includes maintaining accurate records and supporting the wider team to safely manage the waiting lists.
This role requires excellent communication skills both digital and verbal and a confidant manner when speaking to the public.
You will be required to have some knowledge and experience of working with people diagnosed with dementia or other mental illness and be able to offer support and care to them whilst they are attending the group.
You need to be able to work effectively within a team whilst being supported by registered staff.
