Join us as a Band 3 Community Healthcare Support Worker, full-time, in a substantive position. Our service operates from 8am to 8pm, seven days a week. Notts HC also offer an additional team of Nurses and HCA's offer a robust, reactive evening and night Nursing team.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Main duties of the job

The Adult Community Service are an integrated healthcare team providing long-term conditions management, rehabilitation & community nursing services in Arnold.

We are looking for Community Healthcare Support Workers to join our dedicated Community Nursing teams who are committed and passionate about promoting health, well-being and independence. We are looking for a dedicated and enthusiastic clinician who believes in the provision of patient-centred care.

A key aim of our service is to reduce unplanned admissions and facilitate discharges from the acute care environment. The team members include , Community Nurses, District Nurses, Specialist Nurses, Support Workers and Social Workers, also working closely with GPs and the voluntary sector.
