Job summary

We are looking for a Band 5 Community Physiotherapist to join our team.

We are committed & passionate about promoting Health, Well-being and Independence. We are a values-based Trust and have dedicated and enthusiastic clinicians. Do you have a positive and 'can do' attitude? If so...join us! We can provide you with support and development, whilst delivering a fantastic service to our clients.

The position will be covering the Nottingham North and East area. We have home teams based in Carlton, Hucknall and Arnold, with the flexibility to work remotely and from home on occasion. The successful candidate will work within the NNE Community Therapy Team and support the delivery of services such as Complex Community Rehabilitation, Short Term rehabilitation, Discharge to Assess and Long-term conditions management.

Our passion is to maintain independence and keep individuals at home for as long as they are able, avoiding hospital admission and enabling a return home where admission has been unavoidable.

Community Services are the future of Rehabilitation. Working with us will broaden your skill sets and give you the scope and confidence to take your professional development wherever you want it to go!

Main duties of the job

Working within the integrated team, you will be supported to effectively triage, prioritise, assess and provide holistic interventions for clients referred to the service, within their home environment. You will work closely with health and social care colleagues across primary and secondary care to ensure continuity, effective intervention and maximise rehabilitation outcomes.

You will offer bespoke physiotherapy to adults, the frail and elderly including complex fallers, patients with neurological conditions, complex co-morbidities and orthopaedics.

We deliver services between 8am and 8pm over 7 days. We currently operate shift patterns of 8-4pm/12-8pm on 5/7 days.

#TeamNottsHC comprises over 11,000 dedicated colleagues who #MakeADifference every day. We deliver intellectual disability, mental health, community health, forensic, and offender healthcare services across Nottinghamshire, Leicestershire, Lincolnshire, and South Yorkshire. Our care is provided from over 200 sites, spanning community locations, acute settings, and secure environments, including prisons.

We are one of the largest mental health and community Trusts in the East Midlands and one of Nottinghamshire's biggest employers. We also host national and regional services, such as the National High Secure Deaf Service and the Nottingham Centre for Transgender Health.

These networks play a vital role in supporting our diverse workforce and promoting a culture of inclusivity

The health and wellbeing of our colleagues is a top priority. We invest significantly in this through our in-house occupational health and staff counselling services, supported by a dedicated Health and Wellbeing team

The Trust is committed to reducing its carbon emissions, with a specialised Energy and Environmental team working to ensure compliance with environmental legislation, enhance our environmental performance, and achieve our net-zero commitment
