The Mental Health Services for Older People Directorate has an exciting opportunity to join the Transfer of Care Team as a Transfer of Care Facilitator

The Transfer of Care Team manage the pathways of care for patients across the MHSOP Intensive Home Treatment Teams and the inpatient service. The service is responsible for the effective management of all MHSOP inpatient beds located at 2 sites in Nottingham City (Highbury Hospital), Mansfield (Millbrook Mental Health Unit). The wards at Millbrook are currently undergoing redevelopment.

As the Transfer of Care Facilitator you will be responsible for ensuring services continue to run whilst ensuring Delayed Transfer of Care (DTOC) within the Inpatient and Time on Caseload within the IHTT service are monitored, reported and actions completed to resolve any extended stays with the service.

Main duties of the job

You will work collaboratively within a small team, develop and build close working relationships with all community and inpatient teams within MHSOP, other directorates and divisions of Nottinghamshire Healthcare NHS foundation Trust and other statutory and Non-statutory organisations for whom acute bed availability is a key concern.

You will be a highly motivated, driven individual with experience in working within older peoples mental health services. You will have extensive knowledge surrounding the Mental Health Act, Mental Capacity Act and admission/discharge processes between services.
