We are looking for an experienced Learning disability nurse to join our Primary care liaison Team in the City of Nottingham.

Come and join a forward thinking and innovative team to help promote the needs of individuals who have an Intellectual disability and support them and services to enable them get the physical health care they need.

Be part of a team that breakdowns barriers that people with Intellectual Disability experience when accessing primary health care services and support Intellectual disability patients to access National Screening programmes.

Have an active part in delivering enhanced service training to primary care colleagues and supporting the GP Practices in keeping their Learning Disability Registers up to date.

The post is a permanent part - time position.

22.5 hours a week.

Main duties of the job

Liaise with Primary Care, IDD Community Services, Patients, carers and family to help facilitate and promote reasonable adjustments required for people with a Learning Disability to access Primary Health Care.

To educate primary care professionals regarding the Health Needs for people with Learning Disability

To work alongside people with a learning disability so that they are able to access and understand primary care they require.

Support patients to access mainstream screening services.

  • Teaching Qualification

Knowledge

  • An understanding of government policies and legislation relating to people with intellectual and developmental disabilities and their health needs.
  • Knowledge and understanding of current legislation and understanding of clinical risk assessments.
  • An understanding of the application of the Mental Capacity Act. Safeguarding Vulnerable adults and the Equality Act.
  • Knowledge of the nursing 6 C's

Desirable

  • An awareness of the networks and agencies supporting people with intellectual and developmental disabilities operating within the City of Nottingham and the wider county.
  • Awareness of National and Local issues impacting on nursing care

Training

  • Up to date Mandatory and Clinical Training
  • Willingness to undertake new clinical practices, education and training that will enhance service delivery

Experience
