Primary Care Network Occupational Therapist

The closing date is 17 March 2026

Job summary

Do you want to work upstream in changing how we deliver Occupational Therapy in Primary Care? Come to Somerset and be part of an exciting opportunity to contribute to improving population health, through innovation. You may be working in a persons home, GP clinic, in the community or using virtual appointments to offer flexible and accessible provision. All linked to the Network Contract Directed Enhanced Service guidance, which underpins the role of Primary Care Networks.

This role in Taunton Central Primary Care network already has an Occupational Therapist, so the PCNs understand how they wish to expand the current provision for their local populations and the value OT can bring to the team.

As a host model, Primary Care provide daily operational support to delivery and Somerset NHS Foundation Trust support with supervision, professional support and career development.

We anticipate Interviews to be held in February 2026

Main duties of the job

Imagine an Occupational Therapy role where you're able to work as part of a multi-professional team, where your unique skills and contribution are valued and where you can use the full suite of your Core Occupational Therapy skills to both co-create and deliver Occupational Therapy in a team who are focused on maximising health, wellbeing and independence.

This role is based in our Taunton Central Primary Care networks who will offer your operational support; Professional support is delivered by Somerset NHS Foundation Trust.

We believe these roles will be instrumental in achieving the best outcomes for the people living in our communities and would welcome applications for B7 roles (see JD)

The OT will work collaboratively with the PCNs multi-disciplinary team (MDT) to meet the needs of individuals and carers, as well as support the development of new policy and procedures, and provide Occupational Therapy leadership as required.

Our service and roles are embedded fully in Primary Care Multi Disciplinary Teams, focussed on prevention and are in addition to existing provision; although working in close partnership with Neighbourhood services. You will receive support to network into local teams and networks, and may use cutting edge risk stratification tools such as Brave AI.
