We are looking for a dynamic and motivated Occupational Therapist to join our Community Rehabilitation Service based in Frome Community Hospital, Mendip. You will provide rehabilitation in the patients own home assisting them to reach their personalised goals.

Main duties of the job
Provide a high standard of Occupational Therapy within the Rehabilitation services that they are allocated to within Somerset NHS Foundation Trust
Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded
To provide advice regarding the rehabilitation management of patients to families, carers and other members of the
Carry out duties autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the service
Provide support, guidance and training to technical Instructors, therapy support assistants and students

To accept responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities
To carry out assessment and treatments of a range of conditions
To clarify and explain the patient's condition to them and/or their families to enable the formulation of realistic agreed treatment goals and plans. This may sometimes involve the breaking of unwelcome news to the patient and/or their carers
