Job summary

We are seeking a dedicated Registered Mental Health Nurse to join our innovative Mendip Assertive Outreach Team (AOT). In this role, you will conduct comprehensive assessments and deliver psychosocial, evidence-based interventions to individuals experiencing severe and enduring mental health challenges, supporting them on their path to recovery.

You will be part of a dynamic service that has redefined the traditional boundaries between primary and secondary care. Working closely with service users, carers, the voluntary sector, and community enterprises, you will help deliver integrated, person-centred care that makes a real difference.

Main duties of the job

  • Deliver a high-quality, comprehensive mental health service to individuals with complex and enduring needs within the community.

  • Manage a defined caseload, carrying out assessment, planning, implementation, and evaluation of care in collaboration with service users and carers.

  • Provide psychosocial interventions and support to help service users manage daily living, finances, housing, and community engagement.

  • Offer clinical advice, leadership, and supervision to junior staff and students, supporting their professional development.

  • Work collaboratively with inpatient units, CMHTs, HTTs, primary care, and partner organisations to ensure continuity of care.

  • Promote service user involvement in care planning, reviews, and service evaluation.

  • Participate in service development, audit, and quality improvement initiatives.
