Hospital at Home is continuing to expand. Are you a Band 5 Registered Nurse (RN) seeking a new opportunity? We have community nurse positions available within the Hospital at Home service, to cross cover the county of Somerset including travelling to Taunton, Bridgwater and Yeovil. this particular post will be based at Shepton Mallet.
Our service covers the urban and rural areas of West Somerset, from the Quantock Hills to Exmoor. These positions are shifts, working over a 7 day service between 08:00 -- 20:00. Shifts will include weekends, however there are no night shifts.
We offer flexible working solutions where possible to ensure a work-life balance and part-time hours will be considered.
You'll need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Main duties of the job
Hospital at Home, is enabling people to receive acute care and treatments in their home surroundings with support from a team of health and care professionals.
Attend MDT and huddles to discuss patient caseload and plan intervention.
Carry out autonomous assessment and treatment plans for patient within the caseload, whilst managing patient expectations.
Work within scope of practice and seek opportunity for learning from others within team.
Open and honest communication with patients, family and stakeholders.
Prioritise own time and support team to ensure caseload is managed.
Build relationship with unplanned services and neighbourhood teams to support patient to remain within own home.
Work with Internal and external teams to promote Hospital at Home.
Participate in service development, identify own needs and support training to your team and other teams in directorate.
