Job summary
The post holder is suitable for a Registered Nurse, Allied Health Professional, or Paramedic with broad general experience as a qualified professional and the ability to adapt to a wide range of clinical scenarios. The Care Coordination Centre is responsible for the triaging of referrals, managing telephone calls, and overseeing the daily operations of the Centre between 08:00 until 20:00, seven days a week. This service ensures patients receive timely, safe, and appropriate care. The Coordination Centre triages referrals onto unplanned caseloads such as Rapid Response and Hospital at Home, identifies the most appropriate professional to attend, allocates visits, and liaises with clinical staff to maintain continuity of care. The candidate will also signpost patients and/or their carers or relatives to accurate healthcare information, self-care and health improvement advice, or, where applicable, referral to the appropriate level and location of care to meet holistic and therapeutic needs.
Main duties of the job
The post holder will communicate with patients making clinical decisions from information given, obtaining agreement from the patient on the level of care.The post holder will facilitate patient understanding of advice and care decision prior to conclusion of telephone consultation
The post holder will be expected to maintain collaborative, inter-professional links with colleagues, external healthcare professionals and other agencies.The post holder will deal professionally and sensitively with highly personal, delicate, confidential and sometimes complex issues such as bereavement, safeguarding issues.
The post holder will endeavour to control and de-escalate situations which might prevent a safe and effective consultation such as the aggressive patient/carer.The post holder will deal with other barriers to effective communication such as sensory impairment.
The post holder will facilitate the triage process using clinical knowledge and appropriate inter-personal skills demonstrating empathy and understanding. The post holder will triage referrals from SPL both therapy and medical and other sources and prioritise these calls to the Team leaders as appropriate
