Community Nurse

The closing date is 29 January 2026

Job summary

Are you a passionate and motivated nurse ready to make a real difference in patients lives? Join our dynamic Stroke Rehabilitation Unit, where every day brings meaningful progress and inspiring recovery stories.

We are looking for a caring and enthusiastic Band 5 Staff Nurse to join our dedicated multidisciplinary team. This is fantastic opportunity to develop your clinical skills, build strong therapeutic relationships, and be part of a supportive environment that values compassion, teamwork, and professional growth.

In our stroke rehab unit, no two days are the same. You will play a key role in helping patients regain independence, dignity, and confidence through personalised, patient centred care. With access to regular training, mentorship, and development opportunities, you will be supported every step of the way as you build your career.

If you are committed to high quality care, eager to learn, and exited to be part of a team that truly changes lives, this is the role for you.

Main duties of the job

Successful applicants will be responsible for all aspects of patient care, from admission through to discharge, using a computerised record keeping system.

As the patients nurse you will have the opportunity to attend family meetings, take part in the twice weekly stroke consultant ward rounds and be involved in multi-disciplinary team meetings. These all contribute to the decision making process, goal setting and care planning that facilitates a safe discharge home from Hospital.

Alongside this, you will provide general nursing care to all of your patients, ensuring they are treated with dignity and respect, working alongside the multi-professional team. You will be able to promote a safe environment within the recovery centre and in time work as shift coordinator when required.

Previous stroke experience is desirable however not essential as training will be provided wherever needed.
