Job summary
We are looking for a dedicated Senior Mental Health Liaison Worker to join our Community Mental Health Service in Somerset. This is an exciting opportunity to play a key role in transforming how mental health care is delivered across primary and secondary services. You will work within neighbourhood teams and Primary Care Networks, providing early assessment and intervention for adults experiencing mental and emotional distress, including those with complex needs and older adults.The role focuses on breaking down traditional barriers between services, ensuring continuity of care, and promoting best practice in mental health support. Main duties of the job
- Lead assessment, planning, and implementation of interventions for adults (18+) with mental health needs.
- Act as lead professional for a caseload, developing care plans and coordinating treatment.
- Provide structured guidance, supervision, and advice to mental health and voluntary sector staff.
- Build strong relationships with GPs and neighbourhood teams to improve continuity of care.
- Deliver individual and group interventions, promoting best practice in risk management.
- Offer clinical supervision and contribute to staff training and service development initiatives.
- Act as an independent non-medical prescriber, managing medication reviews in line with professional standards.
