Job summary
The Community Rehabilitation Service focus is to provide rehabilitation support to patients within their own communities to achieve what is important to them.
We enable our patients to regain lost skills, increase their levels of independence and wellbeing and to empower and equip them to make lasting changes for their future.
We give them support and guidance to help them to achieve their goals and encourage them to contribute to decisions about their treatment.
Main duties of the job
To work as an integral member of the Community Rehabilitation Service providing effective support to patients through the delivery of tailored rehabilitation programmes devised by Occupational Therapists, Physiotherapists, Assistant Practitioners and Apprentice therapists.
To be able to encourage patients, work with them on their rehab programmes, critically appraise their performance and feedback to them, and the therapists involved, on their achievements. To enable patients to achieve their functional goals and maximise their independence and quality of life.
To work with patients in a range of community locations, including community hospitals, care home rehabilitation units and in service users own homes with overall supervision from rehabilitation professionals.
