Job summary
An exciting opportunity has arisen for a Band 4 Nursing Associate to join our Community Mental Health Service, based at The Bridge.
As a Nursing Associate, you will work alongside Registered Nurses and the wider multi-professional team to assess, plan, and deliver high-quality clinical care that meets both patient and service needs. This role offers the chance to make a real difference by providing holistic, person-centred care to people of all ages, across a range of settings.
You will be registered with the NMC and committed to working in line with the NMC Standards of Proficiency and Code of Conduct. This is an excellent opportunity to develop your skills and experience within a supportive and forward-thinking team.
Main duties of the job
In this role, you will:
Provide safe, evidence-based nursing care to an allocated group of patients, recognising and working within your scope of practice.
Support Registered Nurses in assessing, planning and evaluating care, escalating concerns when required.
Undertake a range of clinical procedures, including medicines management, in line with Trust policy and your competency.
Promote recovery, independence and wellbeing for people with mental health needs, supporting health promotion and illness prevention.
Work as part of the Community Mental Health Service multi-disciplinary team, liaising with colleagues across nursing, medical, therapy, pharmacy and social care.
Support safe and timely patient discharges, helping to reduce hospital readmissions.
Act as an advocate for patients and families, ensuring dignity, equality and inclusion are upheld.
Maintain accurate clinical records and contribute to audit, governance and service improvement initiatives.
Provide support and supervision to Health Care Support Workers, Trainee Nursing Associates and students, helping create a positive learning environment.
