We are seeking a Band 6 Mental Health Practitioner to join our dynamic and forward-thinking Community Mental Health Team (CMHT), working within the West Somerset Primary Care Network (PCN). This role is central to delivering high-quality, person-centred mental health care in a truly integrated, neighbourhood-based model.
You'll be based at Williton Hospital, with regular work in local GP surgeries, collaborating closely with a wide range of professionals across primary, secondary, and voluntary sectors. This is a fantastic opportunity to be part of a service that is breaking down traditional barriers and delivering joined-up care that truly meets the needs of the local population.

Main duties of the job
As a Mental Health Practitioner embedded within the West Somerset PCN, you will play a key role in shaping assessment, formulation, and treatment planning for individuals experiencing a wide range of mental health challenges. You'll provide expert input into care plans, risk management strategies, and direct interventions, while supporting colleagues across the system.
You'll be part of a dedicated PCN hub at Williton Hospital, working alongside a Clinical Director, PCN Manager, pharmacy team, and a well-established social prescribing workforce (including Care Coordinators, Village Agents, and Health Coaches). You'll also be integrated into our innovative 'Living Better' complex care service, collaborating with a Complex Care GP and specialist nursing team.
This role offers a unique opportunity to work in a truly neighbourhood-based service, built on continuity of care, cross-team trust, and shared purpose. Referrals are streamlined through a single point of access, and regular MDTs ensure coordinated, holistic care.
