Job summary
Are you a highly organised, proactive administrator with a passion for patient care and service coordination? Somerset NHS Foundation Trust is looking for a dynamic Integrated Discharge Lead Coordinator to join our dedicated Discharge HUB Team.
Main duties of the job
In this varied and rewarding role, you will be at the heart of patient flow and discharge planning. You'll support our clinical teams, coordinate admin services, manage staff, and act as the key contact for internal and external communication.
You'll play a vital role in service improvement initiatives, CQC readiness, and data tracking -- helping to ensure that patients experience a smooth and safe transition out of hospital.
Key Responsibilities
- Lead and manage a small admin team, including rotas, leave, and performance
- Coordinate communication within the Discharge HUB and with clinical teams
- Maintain accurate discharge data and reports using systems like TrakCare, OPTICA, and Power BI
- Support meetings and documentation for senior managers
- Engage with patients and families during discharge planning
- Uphold confidentiality, compliance, and patient-centred values in all you do
