Job summary

Are you a highly organised, proactive administrator with a passion for patient care and service coordination? Somerset NHS Foundation Trust is looking for a dynamic Integrated Discharge Lead Coordinator to join our dedicated Discharge HUB Team.

Main duties of the job

In this varied and rewarding role, you will be at the heart of patient flow and discharge planning. You'll support our clinical teams, coordinate admin services, manage staff, and act as the key contact for internal and external communication.

You'll play a vital role in service improvement initiatives, CQC readiness, and data tracking -- helping to ensure that patients experience a smooth and safe transition out of hospital.

Key Responsibilities

  • Lead and manage a small admin team, including rotas, leave, and performance
  • Coordinate communication within the Discharge HUB and with clinical teams
  • Maintain accurate discharge data and reports using systems like TrakCare, OPTICA, and Power BI
  • Support meetings and documentation for senior managers
  • Engage with patients and families during discharge planning
  • Uphold confidentiality, compliance, and patient-centred values in all you do

Essential

  • English and Maths GCSE (A* to C grade) or equivalent experience.
  • Ability to demonstrate advanced level of skill across Microsoft Office Suite of software (including Word, Excel, PowerPoint, Outlook).

Desirable

  • Current Knowledge of the Intermediate Care and Discharge Pathways.

Experience

Essential

  • Able to demonstrate skills in problem solving or team or relevant work area.
  • Proven experience or providing customer focussed service.
  • Proven experience in an administrative role.
  • Proven experience of effective communication including with distressed, anxious or angry individuals and able to escalate where appropriate.
  • Ability to give clear instructions to members of the team.

  • Proficient IT skills including keyboard skills and experience of Microsoft packages.
  • Good telephone manner - friendly and approachable.

  • Experience in Health care systems such as White boards, Maxims/Health, Optica, RIO, Trakcare and Power BI where appropriate.
