Job summary
The successful post-holder will offer assessment and treatment for individuals who have severe and enduring mental health problems, this could also be alongside co existing substance misuse disorders. To provide robust and therapeutic treatment as part of the wider community mental health service response to the population of Somerset. We would ideally like the successful applicant to complete the non medical prescribing course but it is not essential.
You will be at the forefront of assessing patients referred to the Older Persons Mental Health Service, using a recovery-oriented approach and strengths model. You will assess, plan, implement and evaluate treatment safely and effectively, in partnership with medical team and nurses, service user and carers, using evidence based practice.
We operate Monday to Friday, but flexibility may be required to meet the needs of our the service. A valid UK driving licence and access to a vehicle is required for the role.
Main duties of the job
The post-holder will work as a key worker within the South Somerset locality ensuring the provision of a high quality service, working with individuals over the age of 65 years supporting with moderate to severe mental health difficulties.
To act as a key worker for a caseload of people with mental health difficulties and developing their care plan, coordinating and reviewing their care and treatment outlined in the plan, at times offering direct individual and group interventions to these and other patients where appropriate
The post-holder will carry a clinical caseload and will provide supervision and advice to other mental health staff in relation to their working practice with this service user group.
