Job summary
The position of Lymphoma Cancer Nurse Specialist exists to meet the needs of patients with lymphoma, ensuring they are placed at the centre of care delivery. Providing specialist nursing advice as clinical expert to support and facilitate high quality, individualised care to patients, relatives and carers within this haematology speciality, in collaboration with multi-disciplinary team.
The post holder will lead the lymphoma nursing workforce, developing and co-ordinating the service, incorporating expert clinical practice, leadership, audit and education.
Main duties of the job
The post holder will work as an integral member of the Multidisciplinary Team within Somerset NHS Foundation Trust and across the community.
The post holder will acknowledge and provide the overall responsibility for the provision of specialist nursing advice and support to patients and families, in-patient and out-patient, throughout their care pathway.
The post holder will utilise skills of clinical expertise, leadership, education, and management to ensure a seamless service for patients, carers and staff. They will provide advice, support and information from the time of diagnosis and throughout care pathway
They will work closely with the multidisciplinary team in hospital and the community to ensure a co-ordinated quality service and will be actively involved in education, audit and research.
The post holder will engage and lead discussions in multidisciplinary team meetings
The ability to work autonomously within the codes of practice and guidelines whilst being accountable for own professional actions
This is a cross county role, with expectation of travel between both acute hospital sites
