Job summary

Are you a newly qualified RMN who is looking for an exciting opportunity to come and work with psychiatric liaison services at Musgrove Park Hospital or a Band 5 clinician looking for a change?

We are offering a 6 month contract covering some maternity leave in a well supported and established team at Psychiatric Liaison Team. In return, we will offer you the opportunity to work alongside experienced PLT clinicians in a multi disciplinary team, where you can learn and develop your skills and abilities in a safe and supported service.

We work with patients in hospital with physical and mental health needs bridging the gap, assessing risks and onward care and support planning.

You will undertake comprehensive Bio Psychosocial assessments and will formulate a range of interventions for patients under the care of Acute Trusts within Somerset, who may be presenting with a suspected or actual mental health problem. You will be guided and supported by senior clinicians on every shift who will support you fully in this role.

Main duties of the job

You will undertake comprehensive Bio Psychosocial assessments and will formulate a range of interventions for patients under the care of Acute Trusts within Somerset, who may be presenting with a suspected or actual mental health problem.

To work alongside senior staff in providing specialist mental health advice to acute staff, enabling them to deal effectively with a range of mental health problems encountered in their clinical areas.

The service focuses on assessment, admission avoidance and discharge planning from A&E and on the facilitation of early but effective discharge for all adults from inpatient wards within the general hospital.

To advance professional knowledge, skills and expertise with the expectation of achieving an advanced level of practice.

To act in a consultative capacity on care issues. Working in partnership with patients and other professionals, to offer specific interventions informed by evidence based or consensus agreed best practice, appropriate to assessed needs, context and culture.

  • *Demonstrate an awareness and knowledge of mental health assessment care planning development and evaluation skills in conjunction with clients and carers.
  • *Knowledge of Mental Health Act legislation
  • *To be conversant with Trust policies and procedures

  • *Experience of managing risk of clients in crisis, recognising when to seek additional supervision
