What's the role?

We are looking for a dynamic and inspiring practitioner to join our Discharge to Assess service, part of the Intermediate Care service for Somerset.

You will work within patients own homes to carry out specific assessments which will include goal setting, treatment planning, assessing risk, promoting and enabling independent functioning for a delegated caseload with the support of qualified staff as required. You will be committed to a person-centred approach to your work, have an ability to prioritise and work flexibly.

Main duties of the job

Who are you?

We are looking for individuals who have extensive experience and knowledge of working in the health and social care setting's. With a broad understanding of conditions and disabilities.

You will manage your own caseload and have the ability to work autonomously as well as working within an integrated team of qualified therapists and providers to meet the needs of your patients.

You will work with patients to identify and set patient led goals to progress independence and identify their longer term care needs.

  • Accredited level 5 occupational competence qualification such as a Foundation Degree, Level 5 Diploma of Higher Education or the Level 5 Diploma for Assistant Practitioners in Healthcare'

Desirable

  • English and Maths at grade C or above

Experience

Essential

  • Experience in community and or acute OT
  • Experience of working with frail and elderly population and providing rehabilitation
  • Supervision and appraisal experience

Desirable

  • Service development experience
