We have an exciting opportunity for a Band 5 or 6 Occupational Therapist to join our Integrated Community Stroke Service in our East Somerset Team, providing inpatient and ongoing community stroke rehabilitation.

The successful applicant will manage a clinical caseload of stroke patients, lead and mentor junior occupational therapists and work alongside a multidisciplinary team, including physiotherapists, Nurses, Speech and Language Therapists, Psychologists, and Rehabilitation Assistants.

Experience in stroke care is essential, as well as excellent communication skills. You will support the development of the stroke occupational therapy workforce, ensuring high-quality care across the county.

The successful applicant will be supported by the Band 7 Clinical Specialist occupational therapist and have access to ongoing development opportunities.

The post is for 37.5 hours per week including bank holidays and weekends, with exact working pattern to be established at the time of appointment.

This post is also open as a 30 hour Band 6 post.

Main duties of the job

*To represent the Stroke Rehabilitation service and/or individual patients at multi-disciplinary/community team meetings, providing specialist information/ advice, to ensure the delivery of a co-ordinated multidisciplinary service, and integrate Rehabilitation into the treatment programme. This will include decision making of patient care, patient progress and involvement in discharge planning.

*To provide written and verbal reports to members of the multi-disciplinary team during treatment and at discharge from the service

*To communicate effectively and work collaboratively with medical, nursing, social services and therapy colleagues to ensure delivery of a co-ordinated multidisciplinary service. This will include providing specialist/ expert advice to case conferences, discharge planning, patient reviews and admission prevention.

*To help maintain good communication, liaison and working arrangement with Trust Directorates, other PCTs, Trusts and other agencies.

  • Membership of COT
  • Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses

Desirable

  • Wheelchair accreditation
  • Post graduate accredited course in Stroke rehabilitation Care of the Elderly and/or Rehabilitation/Neurology.
  • Membership of relevant COT special interest group

Experience

  • Proven post graduate experience with extensive clinical experience relevant to the post
