We have an opportunity for a rehabilitation assistant currently within our service to progress to a Band 4 Assistant Practitioner, as part of our service development and expansion programme. This is an opportunity for a dynamic, motivated and experienced individual to advance their skills to provide a gap in our service provision.

Our ICSS innovative service delivers intensive rehabilitation within patients' homes, clinics and group settings using state-of-the-art mobile technology, ensuring patients are kept at the centre of all decisions, goals, intervention and discharge plans. Our aim is to broaden access to our stroke patients across Somerset and specialist inpatient and ongoing community stroke rehabilitation.

This is an opportunity to help provide patient focused rehabilitative goals in a variety of settings, working with and alongside our Physiotherapy, Occupational Therapy, Nursing and Living Well After Stroke Teams.

You will be required to work 5 days a week, including weekends and bank holidays, to meet the needs of the service.

Main duties of the job

To work as a member of the Multidisciplinary Stroke team to provide Therapeutic interventions for stroke patients, in their own homes and Stroke Rehabilitation Units and clinics

To work with patients and registered members of the team to participate in the assessment, planning and implementation of individualized therapeutic interventions.

To work with patients and registered members of the team to review and update these goals and interventions as appropriate.

To promote and provide holistic person-centered care and support and deliver effective health promotion.

To work along side other agencies as appropriate to provide therapeutic intervention and health promotion.

To identify and provide appropriate assistive equipment for patients to facilitate safety in their chosen environment

To work flexibly to cover the needs of the county wide service, with some shift working required.
